This case highlights the clinical usefulness of three-dimensional (3D) echocardiography. The diagnosis of inter-ventricular septal defect associated with aortic regurgitation has been performed in a 50-year-old man using 3D echocardiography. This advanced echocardiography could accurately reproduce the anatomy of the defect and provide further insights in the mechanisms of aortic regurgitation showing an unusual non-coronary cusp prolapse. The routinely use of 3D echocardiography in clinics might allow a better characterization of cardiac anatomy, especially of aortic valve disorders.

The advanced knowledge about genetic diseases and their mutations has widened the possibility to have a more precise and definitive diagnosis in many patients, but the use of genetic testing is still controversial. Actually, many cardiomyopathies show the availability of genetic testing. The clinical utility of this testing has been widely debated, but it is evident that the use of genetics must be put in a more organic diagnostic pathway that includes the evaluation of risks and benefits for the patient and his relatives, as well as the costs of the procedure. This review aims to clarify the role of genetic in clinics regarding Channelopaties, less frequent but equally important than other Cardiomyopathies because patients can often be asymptomatic until the first fatal manifestation.

Scope of Translational Medicine is to speed the development of new compounds of medical protocols and/or treatments to improve patient’s quality of life. Translational medicine represents the synergy between epidemiology, basic research and clinical trials, and is based upon Innovation Management and Research Development in medicine. Being the speed and progression up to the patient a key issue of Translational Medicine, the innovation process ought to be pursued according to rigorous protocols embedded on a research development path capable of decreasing the lead time at the most. Translational Medicine represents a goal to be pursued by all involved actors, from academic researchers to clinicians, patients and others than can be seen as a network of co-creating actors engaged for the ultimate patient benefit. To underpin Translational Medicine advantages and determinants, the paper approaches the issue by adopting a systems thinking perspective, capable of highlighting the key issues to be considered.

Shoulder arthroplasty is a technically demanding procedure to restore shoulder function in patients with severe osteoarthritis of the glenohumeral joint. The modern prosthetic system exploit the benefits of modularity and the availibility of additional sizes of the prosthetic components. In this paper we describe the biomechanics of shoulder arthroplasty and the technique for shoulder replacement including total shoulder arthroplasty (TSA) with all-polyethylene and metal-backed glenoid component, humeral head resurfacing and stemless humeral replacement.
